Positions at this level work under the supervision of higher level staff to create and/or maintain a service plan throughout the duration of a case. Duties involve conducting assessments; coordinating and delivering direct services with community partners; and documenting all relevant activities, actions taken, and contacts involved with a case.
Knowledge of community and governmental agency human service resources, programs, and services. Knowledge of general office procedures. Ability to develop and maintain effective client relationships. Ability to organize, prepare, and update case files and basic records. Ability to record information and prepare written reports. Ability to problem-solve and manage time efficiently. Ability to operate a motor vehicle and maintain a valid vehicle operator’s license.
Bachelor’s degree. (Substitutions may be allowed.)
